In cases of significant displacement or multiple fragments in the proximal humerus, surgery may be recommended to repair the fracture.

Fixation can be done using pins, plates and screws, or even a prosthetic replacement (hemiarthroplasty or reverse total shoulder replacement), depending on the severity of the injury.
